| Service Description: | Provides outpatient care, education and support to children and adults with type 1 and 2 diabetes, prediabetes and gestational diabetes * team consists of diabetes educators (nurse educators and dietitian educators), social worker, kinesiologist (exercise specialist), endocrinologists, nurse practitioner, and support staff | ||

| Application: | Primary care providers (family doctor or nurse practitioner) or community endocrinologists can refer a patient by contacting the Mississauga Halton Central Intake Program * adults interested in attending a Diabetes Education Program can also contact the Mississauga Halton Central Intake Program at 905-338-2983 ext 4872 to initiate the referral process (self-referral) | ||
