Service Description: Provider of supportive affordable housing and services to people living with HIV/AIDS in the Greater Toronto Area

Addictions Supportive Housing Program - a partnership between Fife House and LOFT McEwan Housing and Support Services
 
AIDS Bereavement and Resiliency Project of Ontario Training and consultation to acquired immune deficiency syndrome service organizations
 
Community Programs Programs created to address isolation and marginalization of men, women and families living with HIV/AIDS * programs include therapies, wellness programs and various food programs
 
Homeless Outreach Program Services to people experiencing homelessness or at risk of experiencing homelessness and living with HIV/AIDS across the City of Toronto * peer-led bi-weekly drop-in for one-on-one housing help
 
Huntley Transitional Housing Program 20-bed transitional housing program designated for people with HIV/AIDS who are chronically homeless and who are accessing the emergency shelter system
 
Ontario Harm Reduction Network (OHRN) Training and consultation to substance use/addiction service agencies in Ontario * research
 
Sherbourne Residence Bachelor, one and two bedroom apartments * 45 units * personal support, homemaking, coordination services
 
Sherbourne Transitional Housing Program Temporary, supportive housing for 9 (nine) months, in a shared communal living environment to 11 people living with HIV/AIDS.
 
Supportive Housing Program Operate Supportive Housing Programs that offer support services:
  • Sherbourne Program - Integrated apartment building with 45 units designated for people living with HIV/AIDS
  • Sherbourne Transitional Housing Program - 11 units of transitional housing with private quarters and communal living spaces and 24/7 support services available
  • Denison Program and Scattered Sites - Residential program for 5 individuals * staff also provide support services to 14 persons at two non-profit housing programs
  • Jarvis Program - 82 unit apartment building
  • Huntley Transitional Housing Program - 20 units of transitional housing for individuals exiting or being diverted from the shelter system.
  • Sorauren Transitional Housing Program - A housing program for women who are survivors of gender based violence.

Eligibility - Population(s) Served: Older adults and adults with disabilities, who need physical, social, or emotional supports * women and men 16 years and older living with HIV infection or AIDS, who require support and are willing to live in a cooperative environment * must be eligible for rent geared to income housing and able to manage personal finances and medication * trans-positive services
