Service Description: The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy.

Medical care team focused on family and community health
  • Offers timely access to high quality primary care
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Administers various health programs
  • Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM)
  • Offers workshops on various topics including but not limited to: Communication and Self Esteem, Coping With Anger and Stress Management
Offers primary health care including but not limited to:
  • Chronic disease management
  • Diabetes program
  • Health promotion
  • Disease prevention
  • Nutritional counselling
  • Mental health clinic
  • Ear syringing
  • Wound care
  • Smoking cessation counselling
