Service Description:
|
An organization that provides respite services to families who are caring for a family member with a physical or developmental disability, a chronic health condition, Alzheimer's disease or a related dementia
Provides a flexible range of respite services for families of children, adults and/or seniors with a disability or a chronic health conditionProvides planned or emergency services in the home or out of the home using either staff or volunteers Host Family Service
- Provides volunteer families who offer periods of respite by welcoming a child or adult with a disability into their home
- Includes the individual in their family activities for a few hours, or an occasional overnight, or a weekend
In-Home Service
- Carries out the in-home care routines that are usually performed by the family caregiver
- Provides the option of taking the family member to activities outside of the home
- Assists participants in the enjoyment of various activities with a focus on their needs, interests and personal safety
Community Friends
- Matches individuals with a disability with volunteers who share their time to enjoy a few hours of social or recreational activities in the community
Respite Home Service
- Operates three respite homes within the City of Thunder Bay
- Engages individuals in daily life routines with small groups of participants
- Provides social and recreational activities tailored to the participants' interests
- Offers weekday and weekend services

Eligibility - Population(s) Served:
|
- The primary caregiver must live in the home with the person receiving the care
- Eligible families are responsible for the ongoing care of the family member
- The family member must have a disability or a chronic health condition such as a developmental disability, autism spectrum disorder, physical disability, frail condition related to aging, Alzheimer Disease or a related dementia
- Specific eligibility is determined by criteria relating to age, diagnosis and location of individuals
Residency Requirements: Districts of Thunder Bay, Kenora, and Rainy River along the corridors of Highways 11/17
