Service Description:	Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers Provides case management services to youth who have either a confirmed diagnosis of a mental illness or are experiencing symptoms of mental illness Offers services that promote the skills youth need to live independently in the community while coping with the symptoms of mental illness

Application:	Can self-refer or be referred by a family member or agency worker Apply through the Northwest Access Point

Eligibility - Population(s) Served:	Youth between the ages of 16-24 Must have a confirmed diagnosis of mental illness or be experiencing symptoms of a mental illness
