Service Description: Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers

  • Delivers sensitive, timely and portable services to individuals who are struggling with a serious mental illness or concurrent disorder
  • Provides a psychosocial model of care to facilitate full and active participation in the recovery process and promote independence
  • Designs services to maximize an individual's functioning in the community
  • Offers case management support services
  • Provides the following services:
    • Assistance in accessing mental health and/or psychiatric services
    • Help to integrate into the community advocating with informal and formal community resources
    • Referrals to community agencies, where appropriate
    • Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program (ODSP)
    • Housing rent supplement
    • Assistance in obtaining and maintaining housing in the community

    • Can self-refer or be referred by a family member, care provider, or agency worker

    • Referrals for case management can be made by visiting Northwest Access Point
