The Sunset Country Family Health Team includes: Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, an Occupational Therapist, Dietitians, Pharmacist, Chiropodist, Social Worker, Consulting Psychiatrist and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy. Supports for individuals living with pre-diabetes or diabetes Offers a diabetes management clinic to assist individuals in understanding and managing their illness Services offered:
