Service Description:	Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers Provides housing for individuals with a serious mental illness or concurrent disorder (mental illness and addiction) Provides subsidized housing and case management services Ensures the individual is living responsibly by providing regular home visits from a case manager Note: Does not provide emergency or crisis housing

Eligibility - Population(s) Served:	Individuals 18 years of age and older with a primary diagnosis of a serious mental illness or concurrent disorder Must be in need of case management services Resident of Thunder Bay Must qualify for Rent Geared to Income Housing
