Service Description:	Operates a self government and is responsible for the day to day operations of the Long Lake 58 First Nation Tribal Council: Matawa First Nations Political Territorial Organization: Nishnawbe-Aski Nation (NAN) Treaty #9 Provides services that promote physical and mental health for community members Provides medical care, health awareness, education and prevention workshops and seminars Coordinates clinics in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases Offers prenatal/postpartum health care, addictions counselling and life skills training Provides smoking cessation counselling Offers First Aid/CPR courses Partners with Dilico Anishinabek Family Care and Matawa Tribal Council to offer local health programs Addresses individual health care needs by providing: Intake services to determine health care needs of individual Referrals to appropriate health care providers based on information in the intake assessment Advocacy on behalf of individual with other health professionals Education and information about Fetal Alcohol Syndrome (FAS) and Fetal Alcohol Effects (FAE) Home visits to the sick, elderly, new mothers and persons with health care needs Assistance with coordinating local and long distance travel arrangements for medical appointments Services and Programs: Aboriginal Healing and Wellness Child and Family Support Program Family Well Being Program Healthy Babies Healthy Children Immunization Clinic Medical Transportation Program National Native Alcohol and Drug Abuse Program
