Service Description:	Coordinates the delivery of health care services to the Anishinabe people across the Sioux Lookout region Operates a primary care medical clinic consisting of 28 family doctors Provides access to other health care professionals within and outside of the Northern Clinic Coordinates documentation if patient receives care from a member of the team Offers primary health care including but not limited to: Chronic disease education and prevention Disease management Health promotion Rehabilitation Telephone Health Advisory Service Provides access to a Registered Nurse through Health Connect Ontario (formerly Telehealth Ontario) who provides health advice and general health information
