| Service Description: |
The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy. Provides a monthly diabetes clinic which offers education and services for the prevention, management and treatment of diabetes
