Service Description:	The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy. Provides mental health services and support Offers case management services including individual assessment and treatment planning Provides supportive mental health counselling to community at large Offers mental health support to all programs of the Family Health Team Provides referrals to local and out of town community service providers Provides individual and public education and information sessions Also provides: Advocacy on various committees and boards for increased mental health programs Individual bereavement and grief counselling

Eligibility - Population(s) Served:	Individuals 16 years of age and older experiencing mental health issues; do not require a formal diagnosis Will provide referrals to required agencies for children younger than 16 years of age
