Service Description: Provides a primary health care clinic and outreach programs for Indigenous individuals and families

  • Promotes independence and self-management for individuals with diabetes through a new incentive program called "Discovering Diabetes"
  • Focuses on early prevention
    • Provides lunch and learn information sessions on topics including:
    • Exercise and diabetes
    • Gestational diabetes
    • Lab and medication review
    • Wellness strategies
    Also offers:
    • Mobile eye van referrals
    • Nutritional guidance
    • Referrals to foot care
