Service Description: Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers

  • Provides case management services to adults living with a mental illness and a developmental disability
  • Promotes recovery and independence while providing appropriate community based support
    • Provides the following services:
    • Assistance in accessing mental health and/or psychiatric services
    • Help to integrate into the community, advocating with informal and formal community resources
    • Referrals to community agencies, where appropriate
    • Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program
    • Housing rent supplement
    • Assistance in obtaining and maintaining housing in the community
