Service Description: | Provides quality supportive housing to single individuals living in the Toronto area living with serious mental illness. Residents must have a formal diagnosis of serious mental illness, be on social assistance, and manage their own medications. They must be able to live somewhat independently or have the desire to live in a "community model" and supportive housing. | ||
Application: | Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. | ||
