Service Description:	Operates as a community-based organization offering primary health care and health promotion programs in areas within the district Provides a variety of clinical services administered by a team of family physicians, nurse practitioners, nurses, foot care nurses, and a dietitian Offers medical care and regular check-ups for patients of the clinic Operates a walk-in clinic for issues such as coughs and cold, fever, earaches, infections, HIV testing, pregnancy test, sexual health, and birth control Offers Telemedicine Services (OTN) Provides referrals to other community services and programs Cervical Cancer Screening Provides cervical cancer screening (HPV testing)

Fees:	None - Most services covered by the Ontario Health Insurance Plan (OHIP)

Eligibility - Population(s) Served:	Must be a patient of the clinic Cervical Cancer Screening Rostered patients and patients without a Health Care Provider
