Service Description: Oxford County Community Health Centre is a local healthcare facility that provides comprehensive, accessible, and affordable medical and support services to individuals and families within our community, especially underserved or vulnerable populations.
Oxford County Community health Centre aims to reduce health disparities, improve public health outcomes, and ensure that everyone—regardless of income, background, or location—has access to essential healthcare services. We play a critical role in preventive care and early intervention, which can lower long-term healthcare costs and improve quality of life.
 
Dietitian Services Nutrition services for all life stages * personal support in group and/or one-to-one sessions with a certified dietitian * programs include:
 
Outreach Outreach staff assist with system navigation, community resources, transportation, advocacy and filling out forms 
 
Youth Outreach and Support Coordinator

The wraparound approach focuses on empowering to identify and work toward goals in:


  • Education and skill building
  • Employment
  • Financial stability and literacy
  • Health and wellbeing
  • Identity, culture and belonging 
  • Recognizing your needs and advocating for them appropriately 
  • Completing paperwork and submitting applications
  • Creating your long-term support network 
  • Referrals to community agencies
  • Working through system challenges





This program is for youth who are:


  • Aged 16-24 
  • Living in Oxford County
  • Feeling overwhelmed by problems and not sure where to start
