Service Description: Provides a primary health care clinic and outreach programs for Indigenous individuals and families

  • Offers education and self-management support to fit an individual's lifestyle
  • Provides access to a Registered Nurse and Registered Dietitian
    • Individual and group diabetes education provided for:
    • Blood glucose monitoring
    • Community Clinic
    • Community Kitchen
    • Foot Care education and examinations
    • Gestational Diabetes education
    • Grocery tours
    • Managing your diabetes
    • Managing diabetes medication
    • Meal planning
    • Nutrition education

    Eligibility - Population(s) Served: Indigenous people 18 years of age and older living in the Rainy River District both on/off-reserve with pre-diabetes, type 1 and type 2 diabetes
