Service Description: An organization that provides community health and wellness by offering primary, traditional, and alternative approaches to care

Supports for individuals living with pre-diabetes or diabetes
  • Provides Certified Diabetes Educators to help individuals manage their diabetes
  • Assesses, monitors, and assists individuals with diabetes management and to attend to any diabetes complications they may have
  • Offers education and self-management support to fit an individual's lifestyle
Offers the following interventions:
  • Complication/Co-morbid condition management
  • Insulin starts and adjustments
  • Healthy Lifestyle
  • Meal Planning
  • Nutritional assessments
  • Pregnancy management
  • Type 1 and Type 2 management
