Service Description: The Points North Family Health Team includes: a Family Physician, Nurse Practitioner, Registered Nurse, and a Social Worker. Family Health Teams allow physicians and other allied health care providers to work as a team to focus on keeping patients healthy.

  • Treats acute and chronic illnesses and provides preventative medicine
  • Provides health education, assessment and treatment for both physical and mental health issues
  • Offers appointments to patients of the clinic whose regular physician is unavailable
  • Provides patients with appointments for urgent, non-life threatening medical issues
