| Service Description: | This program assists with assessing client needs, developing, implementing, and reviewing service plans, and working with other community resources in meeting/achieving the individual's service needs. Support offered: - connect individuals to resources, and advocacy - assess and address substance use concerns - develop and monitor service plans - working with community collaterals to service indivudual's needs - assist individuals to achieve goals and objectives | ||
