| Service Description: | Streamlined Access provides access to mental health and addictions services in York Region and South Simcoe, including Case Management; Assertive Community Treatment Teams (ACTT) and Psychogeriatric Community Treatment Team (PACTT); Regional OHT collaboration Transitional Youth Navigation; Eastern York Region OHT Primary Care Physician referral; single session counselling and navigation pathway and Supports within Housing. Peer support and Family Caregiver Navigator is also available. | ||
