| Service Description: | Provides comprehensive psychiatric treatment and psychosocial rehabilitation. Provides community-based treatment and support. Clients must be heavy users of crisis, emergency departments, and/or inpatient hospital services. | ||
| Application: | Referrals are accepted by anyone, including self-referrals. Referral are through The Access Point. Please call for visit the website. Eligibility Criteria: Individuals diagnosed with a serious mental illness such as schizophrenia, bipolar effective disorder, psychotic disorder and schizoaffective disorder and who have had multiple psychiatric hospitalizations. | ||
