| Eligibility - Population(s) Served: | Adults 18 years and older and have a serious mental illness and significant and complex functional impairment that impacts their ability to live in a community setting * and requires a minimum of 2-3 visits weekly and requires inter-disciplinary care * and require activities of daily living support and a demonstrated history that high level support is required to reduce barriers to living in the community * and meets the threshold for low income (as per HSA) * and be willing to participate in the coordinated care process and agreed upon supports to maintain housing and enhance independent living skills * and be in receipt of sufficient funds to meet financial obligations | ||
