| Service Description: | Specialized case management provides a point of access for individuals: - with a developmental disability and mental health needs and who have complex social, emotional, developmental, behavioural and/or psychiatric needs - who require access to community based developmental / mental health services - who require coordinated and creative service plan to address their immediate need | ||
| Application: | Referrals are through Streamlined Access. Referrals are through Streamlined Access, a referral service in York Region and South Simcoe. Referrals can be made by the individual, their family, and caregiver or from service providers who have obtained consent. Individuals may call or fill out an online application form. | ||
