| Service Description: | Short-term support (3-6 meetings) for individuals transitioning back into the community after clinical treatment for mental health and/or addictions challenges. Offering a “Welcome Basket” of customized items within a limited budget, referrals and connections to community resources, supportive peers to talk to who understand the challenges and can provide support. | ||
