| Service Description: | Provides support within housing for people with mental illness who have come in to contact with the criminal justice system. | ||
| Application: | Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. | ||
