| Service Description: | Provides a continuum of supportive housing to those who are seriously mentally ill and homeless or at risk of becoming homeless. It utilizes a peer support model to encourage natural support networks and includes clients in the decision making process. | ||
| Application: | Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. | ||
