Supports for older adults, and adults with physical disabilities and mental health concerns * helps people remain independent
in their own homes for as long as possible * provides care and support to people who are unable to care for themselves: dementia care, home care, social support, fall prevention, hospice, end of life care and bereavement support * programs to support both the physical and mental wellbeing of people by reducing social isolation and providing care: adult day programs, support groups, nursing care, and friendly visiting programs
