Service Description: Group program running 3-5 days per week * assessment, stabilization, community referral and follow-up for individuals with a diagnosis of serious mental illness
  • groups include: Cognitive Behavioural Therapy, Health and Wellness, Mindfulness, Communication, Self-Esteem, Co-Dependency, Anxiety and Mood, Concurrent Disorders, and Dialectical Behavioural Therapy
  • length of program varies from four to eight weeks
  • individual counselling 
  • family or social support counselling 
  • brief transitional case management for those transitioning from the hospital to the community
Fees: Paid by OHIP
Application: Physician Referral Required (or Nurse Practitioner) - from inpatient unit, psychiatrists, Crisis Intervention Services and Emergency Department
Eligibility - Population(s) Served: Youth, Young Adults, Adults must have a diagnosis of serious mental illness
