| Eligibility - Population(s) Served: |
Persons 16 years old and over with persistent or chronic mental illness
Persons with a high service use as defined by being in an institution on the basis of a mental illness for more than 90 days continuous, 50 days in the past year or 150 days over the past 3 years. Additional consideration is given to clients who exhibit difficulty with homelessness and incarceration Persons with a significant functional impairment demonstrated by: the inability to maintain activities of daily living, the inability to maintain personal safety, and the inability to participate in existing services Complexity of presenting problems given priority, such as coexisting substance use of more than 6 months, co-morbidity of psychiatric diagnosis, or criminal justice involvement |
